Smittlägen

Smittlägen is a term used in epidemiology and infection prevention to describe a situation or setting in which transmission of an infectious agent is possible or likely. The concept captures the conditions under which pathogens can spread between hosts and is used to assess risk and guide preventive measures.

A smittlägen arises from the interaction of three components: the presence of an infectious agent in or around people; the susceptibility and behavior of potential hosts; and environmental and social factors that enable contact or transmission, such as crowding, ventilation, hygiene practices, and routine activities. Transmission can occur through various routes, including direct contact, droplets, airborne particles, and contaminated surfaces (fomites).

Examples of settings that can constitute smittlägen include households with an infected person, healthcare facilities with vulnerable patients, schools and workplaces, mass gatherings, and environments with poor ventilation. The designation of a smittlägen is often dynamic and can change as conditions evolve or as interventions are implemented.

Public health responses aim to reduce transmission in identified smittlägen through measures such as case finding and isolation, contact tracing, hand hygiene, masking where appropriate, improving ventilation, cleaning and disinfection, vaccination, and, if necessary, temporary restrictions or targeted closures. The term is used descriptively to communicate risk and to focus resources during outbreaks.

In Swedish health discourse, smittlägen refers to the current transmission risk context for a given setting or activity and is useful for planning risk-reducing actions.
