ScapulaYAnsicht

ScapulaYAnsicht, or scapula Y view, is a radiographic projection of the shoulder that emphasizes the scapula forming a Y shape. In this view, the acromion and coracoid process form the two upper limbs of the Y, while the body of the scapula forms the stem. The projection is used to assess the relationship of the humeral head to the glenoid and to help identify dislocations and related injuries in the shoulder region.

Technique and positioning are designed to place the scapula in a characteristic orientation for visualization. The patient is rotated obliquely so the scapula lies in a plane that allows the acromion and coracoid to be seen as the arms of the Y, with the humeral head projecting toward the junction of the Y. The arm may be held in a neutral position or with slight rotation, depending on the specific protocol and clinical needs. The X-ray beam is directed perpendicular to the scapula to produce the Y-shaped silhouette.

Interpretation focuses on the alignment of the humeral head relative to the scapular Y. In a normal shoulder, the humeral head sits at the junction of the Y or within the glenoid area. The view is particularly useful for detecting shoulder dislocations and differentiating anterior from posterior displacement, as well as for identifying associated injuries near the glenoid rim. It may also assist in evaluating certain scapular or proximal humeral fractures when other views are inconclusive.

Limitations include the need for proper patient positioning to achieve the Y silhouette and reduced sensitivity for non-acute or subtle lesions. The scapula Y view is typically used alongside standard radiographic views of the shoulder for comprehensive assessment.
