Preeclampsialike

Preeclampsialike refers to a clinical presentation in pregnancy that resembles preeclampsia but may be due to conditions other than true preeclampsia or may not meet formal diagnostic criteria for it. The term is used in some clinical reports and reviews to describe a phenotype characterized by hypertension after 20 weeks’ gestation with signs such as proteinuria or end-organ dysfunction, while the underlying cause differs from classic placental-driven preeclampsia.

Typical features include new-onset hypertension after midgestation, sometimes accompanied by proteinuria or laboratory evidence of organ involvement (elevated liver enzymes, low platelets, renal impairment, or neurologic symptoms). Because several other obstetric and medical conditions can mimic these findings, a thorough evaluation is necessary to identify alternative or coexisting etiologies.

Common causes and differential diagnoses include chronic hypertension with superimposed features, renal disease, autoimmune disorders such as systemic lupus erythematosus or antiphospholipid syndrome, thrombotic microangiopathies (for example TTP/HUS), HELLP syndrome, acute fatty liver of pregnancy, infections, and medication-related effects. Distinguishing these from true preeclampsia is important because it influences management and fetal considerations.

Evaluation typically involves a detailed history, serial blood pressure monitoring, urinalysis for protein, complete blood count, renal and liver function tests, and assessment for thrombotic microangiopathy. Additional tests may be guided by suspected underlying causes (for example, autoantibody panels, ADAMTS13 activity, or renal imaging).

Management centers on treating the underlying condition, controlling maternal blood pressure, and monitoring fetal well-being. If true preeclampsia or severe features are suspected, obstetric management may involve timely delivery. The prognosis and outcomes depend on the underlying cause and gestational age, and there is no single standardized definition for the term preeclampsialike.
