Population-centered is an approach in public policy, public health, and service delivery that places emphasis on the health and well-being of populations rather than solely on individual patients or clients. It defines populations by geography, demographics, risk profiles, or social determinants, and designs interventions and resources to improve outcomes, reduce disparities, and optimize resource allocation.

While related to population health, population-centered practice focuses on planning and evaluating services for groups. It contrasts with person-centered care, which emphasizes individual preferences; in a population-centered approach, collective outcomes and equity guide decisions while individual rights are respected.

Methods and tools include surveillance of health indicators, program planning and evaluation, and the use of epidemiological data, census and administrative records, and geographic information systems. Common metrics include incidence and prevalence, life expectancy, disability-adjusted life years, and measures of inequality such as rate ratios across socioeconomic groups.

Applications appear in vaccination programs, chronic disease prevention, maternal and child health, environmental and occupational health, and urban planning. Data are used to target high-need areas, monitor progress, and reallocate resources as needed.

Challenges involve balancing efficiency with equity, safeguarding privacy, avoiding stigmatization, and ensuring community engagement. Critics warn that focusing on populations may overlook individual needs, while proponents argue that population-level strategies are essential for reducing harm and promoting fair health outcomes.
