The term combines omentum with basing theory, indicating that omental tissue or its derivatives can underpin therapeutic strategies. In this framework, the omentum's rich vascular network, immune cell reservoirs, and capacity for angiogenesis and tissue remodeling are leveraged to promote repair of peritoneal defects, enhance graft integration, and modulate inflammatory responses.

Principles and methods associated with omentalisbased include autologous omental transplantation or flap techniques, processing of omental tissue into cell suspensions or scaffolds, and conditioning of omental tissue to improve survival after transplantation. Applications proposed in early literature include management of peritoneal carcinomatosis, complex abdominal wall defects, and ischemic or inflammatory conditions of the peritoneal cavity.
