In Germany, the Notarzt system is an integral part of the emergency medical services (EMS) structure. When an emergency call is received, dispatchers assess the severity of the situation and, if necessary, activate a Notarzt deployment. The physician, often accompanied by an emergency medical technician (EMT), responds to the scene in a specialized vehicle equipped with advanced medical equipment. This allows for early diagnosis, stabilization, and treatment of patients, which can significantly improve outcomes for conditions such as heart attacks, strokes, severe trauma, or respiratory distress.

The Notarzt system operates under strict protocols to ensure efficiency and coordination with local EMS teams. Physicians are trained in pre-hospital emergency medicine and work closely with paramedics to provide seamless care transitions. While the primary goal is to deliver immediate medical support, the Notarzt may also make decisions about patient transport, such as determining whether to transfer the patient to a specialized hospital or a closer facility based on their condition.
