Nissenfundoplikatio

Nissenfundoplikatio, often referred to as a Nissen wrap, is a surgical procedure primarily used to treat gastroesophageal reflux disease (GERD) and hiatal hernias. It involves wrapping the upper part of the stomach, known as the fundus, around the lower esophageal sphincter. This process strengthens the sphincter, which is the valve between the esophagus and the stomach, preventing stomach acid and contents from flowing back into the esophagus.

The surgery is typically performed laparoscopically, meaning it involves small incisions and the use of a camera and specialized instruments. This minimally invasive approach generally leads to a quicker recovery time, less pain, and reduced scarring compared to traditional open surgery. During the procedure, the surgeon carefully mobilizes the fundus of the stomach and then sutures it around the lower esophagus. There are variations of the Nissen fundoplikatio, such as a partial wrap (e.g., Toupet or Dor fundoplikatio), which may be chosen based on the patient's specific condition and the surgeon's preference.

Post-operative care usually involves dietary modifications, starting with liquids and gradually progressing to solid foods. Patients may experience temporary side effects such as difficulty swallowing, bloating, or gas. While generally effective, the Nissen fundoplikatio is not without potential complications, including recurrence of reflux, dysphagia (difficulty swallowing), or gastric outlet obstruction. The decision to undergo this surgery is made after a thorough evaluation of the patient's symptoms and a discussion of the risks and benefits with their healthcare provider.
