NIVepäonnistuneissa is a Finnish term that translates to "failed NIV" or "NIV failure" in English. It refers to a situation where non-invasive ventilation (NIV) therapy, a method of respiratory support that does not require intubation, is not effective in improving a patient's condition or adequately addressing their respiratory distress.

NIV is commonly used for conditions such as acute exacerbations of chronic obstructive pulmonary disease (COPD), acute cardiogenic pulmonary edema, and as a bridge to mechanical ventilation in some cases. When NIV fails, it means that despite the application of positive pressure ventilation through a mask or other interface, the patient's breathing remains inadequate. This can manifest as persistent hypoxemia (low blood oxygen levels), hypercapnia (high blood carbon dioxide levels), increased work of breathing, or a worsening clinical status.

Reasons for NIV failure can be multifactorial. These may include patient intolerance to the interface, air leaks, excessive secretions, underlying disease severity that is beyond the scope of NIV, or failure to identify and treat the underlying cause of respiratory failure. In such instances, escalation of care is typically required, which may involve transitioning to invasive mechanical ventilation via endotracheal intubation or adjusting medical management for the underlying condition. Identifying NIV failure promptly is crucial for timely intervention and improving patient outcomes.
