Lofstrand

Lofstrand, commonly referred to as a forearm crutch, is a mobility aid used by people who have limited ability to bear weight on the legs. It consists of a handgrip, a vertical shaft, and a forearm cuff that wraps around the lower part of the arm just below the elbow. The user bears weight through the hands and forearms, while the cuff provides proximal support and alignment for the crutch.

The device differs from axillary crutches by eliminating contact under the armpit. This design can reduce neck and shoulder strain and allows for more trunk mobility, but it requires greater upper-body strength, coordination, and balance. Lofstrand crutches are commonly prescribed after injuries or surgery and for certain chronic conditions that affect lower-limb strength or stability, such as osteoarthritis or neurological disorders. They are often used for longer-term rehabilitation or disability management, depending on individual needs.

Fit and use are important for safety and effectiveness. Proper height adjustment places the elbow in a slight flexion when bearing weight, typically around 20 to 30 degrees. The cuff should be snug but not constrictive, and the handgrip should be comfortable to hold while maintaining a neutral wrist position. Users should be mindful of distributing weight through the hands rather than the armpits and should avoid leaning on the cuff to prevent nerve or tissue irritation.

Care considerations include inspecting the cuff and shaft for wear, ensuring rubber tips are intact, and keeping the device clean. If numbness, tingling, or weakness occurs, a clinician should reassess fit and technique.
