The JAK2V617F mutation is a specific genetic alteration found in the Janus kinase 2 (JAK2) gene. This mutation is a point mutation, meaning a single nucleotide change in the DNA sequence, resulting in the substitution of valine with phenylalanine at amino acid position 617. It is a somatic mutation, meaning it occurs after conception and is present in some but not all cells of an individual.

This mutation is a key driver in the development of certain myeloproliferative neoplasms (MPNs), a group of blood cancers characterized by the overproduction of one or more types of blood cells. Specifically, the JAK2V617F mutation is found in approximately 95% of patients with polycythemia vera, about 50-60% of patients with essential thrombocythemia, and about 50% of patients with primary myelofibrosis.

The presence of this mutation leads to constitutive activation of the JAK-STAT signaling pathway. This pathway plays a critical role in cell growth, differentiation, and survival. When the JAK2V617F mutation is present, the JAK2 protein is constantly active, even in the absence of normal growth factor signals. This dysregulated signaling promotes the uncontrolled proliferation of hematopoietic stem cells, leading to the characteristic blood cell overproduction seen in MPNs. Genetic testing for the JAK2V617F mutation is a standard diagnostic tool for individuals suspected of having these conditions and can help guide treatment decisions.
