Implantatlösning, also known as implant failure or implant dehiscence, refers to the detachment or separation of a dental implant from its surrounding bone. This can occur at various stages, from immediately after placement to years later. The primary cause is typically a lack of osseointegration, the process by which the bone fuses with the implant surface.

Several factors can contribute to implantatlösning. Peri-implantitis, an inflammatory condition affecting the tissues around the implant, is a common culprit. This inflammation can lead to bone loss and compromise the stability of the implant. Poor oral hygiene, smoking, uncontrolled diabetes, and certain systemic diseases can increase the risk of peri-implantitis. Mechanical overload, such as excessive biting forces or grinding (bruxism), can also stress the implant and lead to failure, especially if the implant is not adequately supported by bone.

Other contributing factors include inadequate primary stability of the implant during surgery, poor implant placement technique, and the presence of infection at the surgical site. In some cases, issues with the implant material or design may also play a role.

The signs and symptoms of implantatlösning can vary. They may include pain, swelling, bleeding, pus discharge from the implant site, and loosening of the implant or restoration. In some instances, the implant may become mobile. Diagnosis usually involves clinical examination, radiographic assessment (X-rays), and sometimes probing of the implant sulcus to assess bone levels. Treatment for implantatlösning depends on the cause and severity. It can range from conservative measures like improving oral hygiene and treating peri-implantitis to more invasive procedures such as surgical debridement, bone grafting, or ultimately, implant removal.
