Fetusbewaking

Fetusbewaking, in obstetrics, refers to the ongoing surveillance of a fetus during pregnancy and labor to assess its wellbeing and detect signs of distress. The practice combines fetal heart rate monitoring with assessment of uterine activity and other indicators of fetal status.

Common methods include electronic fetal monitoring (EFM) or cardiotocography (CTG), which continuously records fetal heart rate and contractions, and intermittent auscultation using a Doppler device or fetoscope. Additional tests such as the non-stress test (NST), biophysical profile (BPP), and contraction stress test (CST) may be used to further evaluate fetal well-being.

Fetusbewaking is most commonly employed in high-risk pregnancies, during labor, or when there are concerns about placental function, fetal growth, or maternal conditions such as hypertension or diabetes. It can also be used routinely in some settings.

Benefits include the ability to identify fetal distress and guide clinical decisions to protect the fetus, such as augmentation of labor or cesarean delivery. Limitations include false alarms, variability in interpretation, and the possibility that reassuring monitoring does not guarantee a favorable outcome. Over-reliance on monitoring without clinical context may lead to unnecessary interventions.

The approach has evolved since the mid-20th century, with ongoing research into improving predictive value and reducing unnecessary interventions. Practices vary by country, clinical setting, and patient risk profile.
