FMEAanalyysi, or Failure Mode and Effects Analysis, is a systematic, proactive method for evaluating a process or product to identify potential failure modes and their causes and effects. The goal is to identify and eliminate or reduce these failures before they occur, thereby improving reliability and quality.

The FMEA process typically involves a cross-functional team that reviews the design or process. They brainstorm potential ways a product or process could fail (failure modes), the potential causes of these failures, and the effects if these failures were to occur. For each identified failure mode, a risk assessment is performed, often involving assigning numerical ratings for severity, occurrence, and detectability. These ratings are then used to calculate a Risk Priority Number (RPN), which helps prioritize which failure modes to address first.

Based on the RPN and the team's judgment, actions are developed and implemented to prevent or mitigate the highest-priority risks. The effectiveness of these actions is then monitored, and the FMEA is updated as necessary. FMEA can be applied to various contexts, including product design (Design FMEA or DFMEA) and manufacturing processes (Process FMEA or PFMEA), and is a valuable tool in quality management and risk assessment.
