Aistiharhoja

Aistiharhoja is a Finnish term describing perceptual disturbances in which senses are misperceived or experiences occur without external stimuli. They can involve any sense: visual, auditory, tactile, olfactory, or gustatory. Visual aistiharhoja may include flashes, shapes, or figures that are not present; auditory ones may consist of voices or sounds; tactile experiences can feel like crawling or tingling; olfactory and gustatory episodes involve smells or tastes without a source.

Aistiharhoja occur in a variety of contexts and may reflect different underlying causes. They can appear in psychiatric disorders such as schizophrenia, bipolar disorder with psychotic features, and major depressive disorder with psychotic symptoms. Neurological conditions like epilepsy, migraines, Parkinson’s disease, or brain tumors can also produce them. Substance use or withdrawal (for example alcohol, stimulants, or hallucinogens) is another common cause. They can also arise from severe sleep deprivation, high fever, sensory deprivation, or metabolic disturbances.

Assessment typically relies on clinical interview and medical history, with attention to timing, triggers, and accompanying symptoms. Distinguishing true hallucinations from misperceptions (illusions) or dreams is important. Diagnostic manuals such as DSM-5 or ICD-11 provide criteria for related conditions, but evaluation often requires interdisciplinary care.

Treatment focuses on the underlying cause. Addressing medical or psychiatric conditions can reduce or resolve aistiharhoja. Antipsychotic or other psychotropic medications may be prescribed, along with psychotherapy, sleep management, and supportive care. Education for patients and families aids coping and safety.
