Tractography is a neuroimaging technique used to visualize and map the white matter tracts in the brain. It is a three-dimensional modeling method that uses data from diffusion tensor imaging (DTI), a specialized form of magnetic resonance imaging (MRI). DTI measures the diffusion of water molecules within brain tissue, which tends to move more freely along the length of nerve fibers than in other directions. By analyzing these directional differences, tractography reconstructs the pathways of white matter tracts, providing insights into brain connectivity.

The process begins with acquiring DTI scans, which capture the diffusion of water in multiple directions. These scans generate a diffusion tensor at each voxel (a three-dimensional pixel) in the brain, representing the direction and anisotropy (directional dependence) of water diffusion. Tractography algorithms then trace these tensors to infer the pathways of white matter fibers, often using techniques like deterministic or probabilistic tracking. Deterministic methods follow the principal eigenvector of the diffusion tensor, while probabilistic methods account for uncertainty by sampling multiple possible pathways.

Tractography has applications in both research and clinical settings. In neuroscience, it helps study brain connectivity, developmental changes, and the effects of neurological disorders. Clinically, it aids in diagnosing and planning treatments for conditions such as multiple sclerosis, stroke, brain tumors, and traumatic brain injury. By visualizing disrupted or altered white matter pathways, tractography can provide valuable information for surgical planning, rehabilitation strategies, and understanding disease progression.

While tractography is a powerful tool, its results depend on the quality of the imaging data and the choice of algorithms. Challenges include partial volume effects, crossing fibers, and the interpretation of indirect measurements.
