Tonguetied is a condition characterized by difficulty in speaking due to a short or tight lingual frenulum. The lingual frenulum is a piece of tissue that connects the underside of the tongue to the floor of the mouth. When this tissue is abnormally short, thick, or inflexible, it can restrict the tongue's movement, leading to problems with speech articulation, eating, and swallowing.

The severity of tongue-tie, medically known as ankyloglossia, can vary. Mild cases may cause no significant issues, while more severe cases can result in speech impediments such as difficulty pronouncing certain sounds like 't', 'd', 'z', 's', 'th', 'r', and 'l'. This can manifest as lisps or unclear speech. In addition to speech problems, tongue-tie can also affect breastfeeding for infants, making it difficult to latch properly, which can lead to poor weight gain and discomfort for the mother. In older children and adults, it can contribute to issues like a limited range of motion for the tongue, which might interfere with eating certain foods, and in some cases, may lead to dental problems like gaps between the lower front teeth or difficulty cleaning the teeth.

Diagnosis is typically made by a medical professional, such as a pediatrician, dentist, or speech-language pathologist, through a visual examination of the tongue and assessment of its mobility. Treatment options depend on the severity of the condition and the impact it has on the individual's life. For mild cases, observation may be sufficient. For more significant cases, a simple surgical procedure called a frenotomy may be recommended. This involves cutting or releasing the lingual frenulum to allow for greater tongue movement. Speech therapy may also be used to help improve articulation skills following the procedure.
