Thiouracils are a class of organic compounds that are derivatives of uracil, a pyrimidine base found in RNA. They are characterized by the presence of a sulfur atom replacing one of the oxygen atoms in the uracil ring, typically at the 2- or 4-position. The most well-known examples are propylthiouracil (PTU) and methimazole, although methimazole is technically an imidazole derivative and often grouped with thiouracils due to its similar mechanism of action.

These compounds have been historically significant in medicine, primarily for their use as antithyroid drugs. They function by inhibiting the enzyme thyroid peroxidase, which is crucial for the synthesis of thyroid hormones (thyroxine and triiodothyronine). By blocking this enzyme, thiouracils reduce the production of these hormones, making them effective in managing hyperthyroidism, a condition where the thyroid gland produces too much hormone.

While once widely used, the therapeutic application of thiouracils has seen some shifts due to the development of other antithyroid agents and concerns about potential side effects. Propylthiouracil, in particular, has been associated with a risk of severe liver damage, leading to restrictions on its use in some regions. Nevertheless, they remain important in specific clinical scenarios, such as during the first trimester of pregnancy or in cases of thyroid storm, where their rapid onset of action can be beneficial. Research continues to explore their properties and potential applications beyond their established role in thyroid management.
