Theischial is an anatomical term meaning relating to the ischium, one of the three bones that form the pelvis. The ischium contributes to the lower, posterior portion of the hip bone and, with the ilium and pubis, forms the acetabulum and contributes to the pelvic ring. The body of the ischium helps form the posterior wall of the acetabulum, while its ramus extends downward and forward to join the inferior ramus of the pubis, creating the ischiopubic ramus.

Key features in the ischial region include the ischial tuberosity, a prominent rough projection that bears weight when a person sits; the sacrotuberous ligament attaches to the tuberosity. The ischial spine is a pointed projection above the greater sciatic notch, and the sacrospinous ligament attaches near this area. The greater and lesser sciatic notches lie along the posterior border of the ischium and, with surrounding ligaments, form the greater and lesser sciatic foramina.

Muscular attachments: The hamstring muscles—the semitendinosus, semimembranosus, and the long head of the biceps femoris—originate from the ischial tuberosity. The ischial spine serves as the origin for the superior gemellus, while the inferior gemellus attaches nearby on the ischial tuberosity. The ischial region also provides attachments related to the hip joint capsule and nearby deep gluteal structures.

Clinical relevance: The ischial tuberosities bear seating weight, making them a common site for pressure ulcers in immobile individuals. Avulsion fractures of the ischial tuberosity can occur in young athletes during rapid hip flexion with knee extension. Ischiogluteal bursitis is another condition affecting the area and can cause buttock pain.

Notes: The form theischial is uncommon in modern anatomy; most texts use ischial as the standard adjective. The term may appear in older literature or as part of compound forms.
