Subacromial refers to the space located beneath the acromion, which is a bony projection from the shoulder blade. This space is a crucial anatomical area within the shoulder joint, housing several important structures. Primarily, it contains the supraspinatus tendon, which is one of the four tendons of the rotator cuff. Also found within this space are the subacromial bursa, a fluid-filled sac that reduces friction, and portions of the long head of the biceps tendon.

The subacromial space is narrow, and its dimensions can vary between individuals. Conditions affecting this space often lead to pain and restricted movement in the shoulder. Subacromial impingement syndrome is a common condition where the structures within this space become compressed or pinched during arm elevation. This can be caused by inflammation of the bursa (subacromial bursitis), degeneration or tears of the rotator cuff tendons, or bone spurs on the acromion.

Symptoms associated with subacromial issues typically include pain at the front or side of the shoulder, which may worsen with overhead activities or lying on the affected side. Weakness in the shoulder and a reduced range of motion are also common. Diagnosis often involves a physical examination, imaging tests such as X-rays or MRI, and sometimes diagnostic injections. Treatment strategies vary depending on the cause and severity, and can range from conservative measures like rest, physical therapy, and medication to surgical interventions aimed at decompressing the space or repairing damaged tissues.
