Bisphosphonates are a common class of drugs used to treat osteoporosis. They work by inhibiting osteoclasts, the cells responsible for breaking down bone tissue. Examples include alendronate, risedronate, and zoledronic acid. These are available in oral and intravenous forms.

Another category of medications includes bone-building agents, also known as anabolic agents. These drugs stimulate osteoblasts, the cells that build new bone. Teriparatide and abaloparatide are examples of these, typically administered via injection. They are often used for severe osteoporosis or when other treatments have not been effective.

Hormone replacement therapy (HRT) can also be used, particularly in postmenopausal women, to prevent bone loss by replacing estrogen. However, HRT carries potential risks and is usually considered for women with significant risk factors for osteoporosis and menopausal symptoms.

Selective estrogen receptor modulators (SERMs) like raloxifene mimic the beneficial effects of estrogen on bone without some of the risks associated with traditional HRT. Denosumab is a monoclonal antibody that works by blocking a protein that signals osteoclasts to break down bone. It is administered via injection every six months.

The choice of medication depends on individual factors such as the severity of osteoporosis, the patient's medical history, and potential side effects. Regular monitoring and lifestyle modifications, including diet and exercise, are also crucial components of osteoporosis management.
