Nociception, sometimes written as nociceptioon, refers to the neural processes by which potentially harmful stimuli are encoded and transmitted from peripheral receptors to the central nervous system. It is the physiological basis for the sensation of pain, but pain itself is a subjective experience influenced by emotion, context, and cognition.

Nociceptors are specialized sensory endings located in skin, joints, visceral tissues, and some other areas. They respond to mechanical, thermal, or chemical stimuli, and some are polymodal, capable of detecting multiple noxious modalities. Activation of nociceptors generates action potentials that travel via two main types of primary afferent fibers: A-delta fibers, which convey fast, sharp, well-localized pain, and C fibers, which convey slower, dull, aching pain. These signals enter the spinal cord and ascend to higher brain centers through pathways such as the spinothalamic tract.

Within the central nervous system, nociceptive information is processed at the dorsal horn and can be modulated by local circuits and descending pathways. Descending pain modulatory systems originate in brain regions including the periaqueductal gray and the rostral ventromedial medulla, capable of dampening or enhancing nociceptive signaling. Peripheral sensitization after tissue injury lowers nociceptor thresholds, while central sensitization can amplify pain signaling within spinal and supraspinal circuits.

Clinically, nociception may be present without conscious pain, as under anesthesia, and chronic pain conditions often involve altered nociceptive processing. Analgesic strategies frequently target different steps of the nociceptive pathway, from peripheral receptors to central modulation.
