A nerve stimulator is a medical device that delivers electrical pulses to targeted nerves or neural pathways to modify their activity for therapeutic purposes. The term covers implantable neuromodulation systems and non-invasive stimulators used in a variety of conditions.

Implantable systems include peripheral nerve stimulators, spinal cord stimulators, vagus nerve stimulators, and sacral nerve stimulators. These devices typically consist of a pulse generator (a compact battery-powered unit) placed under the skin and one or more leads that are positioned near the target nerve or spinal region. Non-invasive options, such as transcutaneous electrical nerve stimulation (TENS) devices, use external power sources and surface pads to deliver stimulation.

Mechanism and use: Electrical stimulation can alter nerve signaling and brain networks. Pain-modulating devices often activate larger nerve fibers to inhibit pain signals at the spinal cord level or within central pain networks. Vagus nerve stimulation influences neural circuits in the brain and related neurotransmitter systems. Spinal cord and peripheral nerve stimulators modulate signaling along specific pathways to reduce symptoms.

Clinical indications: Nerve stimulators are used for chronic pain management (including neuropathic pain and certain post-surgical pain conditions), complex regional pain syndrome, and other refractory pain states. Sacral nerve stimulation treats overactive bladder and certain fecal incontinence conditions. Vagus nerve stimulation is approved for refractory epilepsy and treatment-resistant depression, with occipital nerve stimulation explored for chronic migraines.

Procedure and safety: Before permanent implantation, a trial stimulation period is commonly performed to assess efficacy. If successful, a generator is implanted and connected to leads near the target nerve. Battery life typically lasts several years, with replacement surgeries as needed. Risks include infection, lead migration or fracture, hardware malfunction, and limitations related to MRI compatibility.

Regulatory status and evidence: Nerve stimulators require regulatory approval in their region (for example, FDA in the United States, CE marking in the European Union). Evidence supports benefit for selected patients, but results vary and devices are not curative.
