Myotonischen is the German adjective form related to myotonia, a neuromuscular sign characterized by delayed muscle relaxation after voluntary contraction. In medical contexts it is used to describe symptoms, syndromes or disorders in which myotonia is prominent. Myotonia can be present from birth or develop later and may occur as an isolated sign or as part of a broader muscular disorder.

Causes and classification include genetic channelopathies and muscular dystrophies. The best known forms are myotonia congenita, caused by mutations in the CLCN1 gene and presenting with muscle stiffness that improves with repeated activity, and the myotonic dystrophies type 1 and type 2, which are multisystem disorders due to different genetic defects (for example DMPK/CNBP gene expansions). Other forms involve mutations in sodium channels (SCN4A) or other ion-channel genes. These conditions share the feature of myotonia but differ in age of onset, distribution of weakness, and associated systemic findings.

Pathophysiology, at a general level, involves impaired muscle fiber repolarization and altered electrical excitability, often linked to reduced chloride conductance or abnormal ion-channel function. This leads to delayed relaxation, stiffness and sometimes myotonic discharges detectable on electromyography. A characteristic warm-up phenomenon may decrease myotonia after brief exercise.

Diagnosis relies on clinical observation, electromyography showing myotonic discharges, and genetic testing to identify specific mutations. Management is supportive and individualized, emphasizing physical therapy and stretching to maintain flexibility, and, in some cases, pharmacological agents such as mexiletine or other sodium-channel blockers to reduce myotonia. Cardiac and respiratory involvement in systemic forms requires appropriate monitoring and treatment. Prognosis varies with the underlying condition and extent of multisystem involvement.
