Leptomeningeal refers to the two innermost membranes that surround the brain and spinal cord, collectively known as the meninges. These two membranes are the pia mater and the arachnoid mater. The pia mater is a delicate, thin membrane that closely adheres to the surface of the brain and spinal cord, following all its contours. The arachnoid mater is a more fibrous and web-like membrane situated external to the pia mater. Between the arachnoid mater and the pia mater is the subarachnoid space, which is filled with cerebrospinal fluid (CSF). This fluid plays a crucial role in cushioning and protecting the central nervous system.

The term "leptomeningeal" is often used in the context of medical conditions affecting these membranes. For example, leptomeningeal disease refers to the spread of cancer cells to the leptomeninges. This condition, also known as leptomeningeal carcinomatosis or leptomeningeal metastasis, can arise from primary tumors in the brain or from cancers that have spread from elsewhere in the body. Symptoms can vary widely but may include headaches, neurological deficits, seizures, and cognitive changes, depending on the location and extent of the involvement. Diagnosis typically involves imaging techniques such as MRI and analysis of cerebrospinal fluid obtained through a lumbar puncture. Treatment strategies aim to control tumor growth and manage symptoms, and can include radiation therapy, chemotherapy, and targeted therapies.
