This extensive process aims to identify and synthesize new chemical entities or biologics that could potentially treat diseases. Once a target is identified, researchers employ various methods to find molecules, known as lead compounds, that interact with this target. These methods can include high-throughput screening of large chemical libraries, rational drug design based on structural biology, or studying natural products. Following the identification of lead compounds, medicinal chemists optimize their structure to improve efficacy, reduce toxicity, and enhance pharmacokinetic properties, making them suitable for further development.

The optimized compounds then undergo rigorous preclinical testing in laboratory settings, using cell cultures and animal models, to assess their safety and effectiveness before human trials can begin. If preclinical results are promising, the drug candidate progresses to clinical trials, which involve testing in human volunteers in multiple phases to confirm safety, dosage, efficacy, and side effects. Successful completion of all clinical trial phases is required for regulatory approval by health authorities, such as the European Medicines Agency (EMA) or the U.S. Food and Drug Administration (FDA), before a drug can be marketed to the public.
