Kosketustekijät, or contact factors, are a group of proteins that play a crucial role in the intrinsic pathway of the blood coagulation cascade. These factors are activated when blood comes into contact with negatively charged surfaces, such as damaged blood vessel walls or certain laboratory materials. The primary kosketustekijät include Factor XII (Hageman factor), prekallikrein, and high molecular weight kininogen (HMWK).

The activation of the kosketustekijät system initiates a cascade of enzymatic reactions. When Factor XII encounters a suitable surface, it undergoes autoactivation or is activated by other molecules. Activated Factor XII (Factor XIIa) then cleaves prekallikrein to form kallikrein. Simultaneously, Factor XIIa can also cleave Factor XI to initiate the common pathway of coagulation. Kallikrein, in turn, can further activate Factor XII and cleave HMWK, releasing bradykinin, a peptide with vasodilatory and inflammatory properties.

While the kosketustekijät system is intrinsically linked to blood clotting, its precise physiological role in normal hemostasis in vivo is still a subject of ongoing research. Individuals with deficiencies in Factor XII, prekallikrein, or HMWK often do not exhibit significant bleeding disorders, suggesting that other pathways are more dominant in preventing excessive blood loss. However, these factors are implicated in various pathological processes, including thrombosis, inflammation, and angioedema. Their interaction with the fibrinolytic system and the kallikrein-kinin system highlights their broader involvement in vascular homeostasis and inflammatory responses. Research continues to explore the therapeutic potential of targeting the kosketustekijät system for the management of thrombotic and inflammatory diseases.
