Granulocytopenia is a condition characterized by a lower-than-normal number of granulocytes in the blood. Granulocytes are a type of white blood cell that play a crucial role in the immune system, primarily by fighting off bacterial and fungal infections. They are characterized by the presence of granules in their cytoplasm, which contain various enzymes and proteins. The main types of granulocytes include neutrophils, eosinophils, and basophils.

Neutrophils are the most abundant type and are the first responders to infection. A significant decrease in neutrophils, known as neutropenia, is the most common cause of granulocytopenia and leads to an increased risk of infection. Eosinophils are involved in allergic reactions and fighting parasitic infections, while basophils release histamine and other mediators in allergic responses.

The causes of granulocytopenia are diverse. They can include autoimmune disorders where the body attacks its own granulocytes, certain medications (especially chemotherapy drugs and some antibiotics), viral infections that suppress bone marrow function, and bone marrow disorders such as aplastic anemia or leukemia. Severe infections can also sometimes lead to a temporary drop in granulocyte counts as they are consumed in fighting the infection.

Symptoms of granulocytopenia are often related to the increased susceptibility to infection. These can include fever, chills, sore throat, cough, frequent or persistent infections, and skin infections. Diagnosis is typically made through a complete blood count (CBC) which reveals a low absolute granulocyte count. Treatment strategies depend on the underlying cause and severity, and may involve managing the underlying condition, discontinuing offending medications, or in severe cases, using growth factors to stimulate the bone marrow to produce more granulocytes.
