Exacerbatie is a medical term describing a sudden or progressive worsening of a disease or its symptoms, resulting in increased severity and impact on function. The word originates from Latin roots meaning to aggravate or intensify. In medical language, it often refers to a temporary deterioration that differs from normal fluctuations in chronic conditions.

The concept is commonly used for chronic diseases such as chronic obstructive pulmonary disease (COPD), asthma, inflammatory bowel disease, heart failure, and autoimmune disorders. In COPD, an acute exacerbation is defined by a notable worsening of dyspnea, cough, and/or sputum production that persists for more than 24 to 48 hours and may require a change in therapy. In asthma, an exacerbation denotes a period of intensified symptoms that may necessitate additional inhaled medications or systemic steroids. Exacerbations can also occur in conditions like rheumatoid arthritis or multiple sclerosis, presenting as a flare in symptoms or activity.

Exacerbation must be distinguished from relapse (return of symptoms after a period of improvement) and from disease progression (gradual worsening over time). Triggers commonly include infections, environmental pollutants, nonadherence to treatment, and comorbid conditions. Diagnosis relies on clinical assessment, and in some diseases may involve objective tests such as spirometry, imaging, or laboratory studies.

Management focuses on addressing triggers, optimizing baseline therapy, and providing supportive care. This can include bronchodilators, corticosteroids, antibiotics when infection is suspected, oxygen therapy, and hospitalization for severe cases. Preventive strategies—such as vaccination, smoking cessation, medication adherence, and early symptom recognition—aim to reduce frequency and severity of future exacerbations.
