Endotelafhængighed refers to the physiological processes regulated by the endothelium, the inner lining of blood vessels. The endothelium plays a crucial role in maintaining vascular homeostasis and is a dynamic endocrine organ. Endothelial cells produce and release a variety of vasoactive substances that influence blood flow, coagulation, inflammation, and cell growth.

One key function of the endothelium is the production of nitric oxide (NO). NO is a potent vasodilator, meaning it relaxes blood vessels, thereby lowering blood pressure and improving blood flow. It also inhibits platelet aggregation and the adhesion of leukocytes to the vessel wall, contributing to anti-thrombotic and anti-inflammatory effects.

Another important aspect of endotelafhængighed is the regulation of vascular tone. Endothelial cells can release vasoconstrictors, such as endothelin-1, which narrow blood vessels, and vasodilators like prostacyclin, which widen them. The balance between these substances is critical for maintaining appropriate blood pressure and organ perfusion.

Dysfunction of the endothelium, often termed endothelial dysfunction, is an early event in the development of many cardiovascular diseases, including atherosclerosis, hypertension, and diabetes. This dysfunction can manifest as reduced NO bioavailability, increased production of reactive oxygen species, and an altered response to stimuli, leading to a pro-inflammatory and pro-thrombotic state. Assessing endothelial function is therefore important in clinical practice for risk stratification and management of cardiovascular conditions.
