Edemaare refers to edema that cannot be readily explained by a single mechanism, reflecting interaction among increased capillary hydrostatic pressure, reduced plasma oncotic pressure, and impaired lymphatic drainage. The concept emphasizes the multifactorial nature of fluid accumulation and the potential for simultaneous involvement of legs, abdomen, and thoracic compartments in a single patient.

Causes and pathophysiology

Conditions that can contribute to an edemaare phenotype include chronic venous insufficiency, heart failure, nephrotic or hepatic disease, inflammatory states, and primary or secondary lymphatic dysfunction. Pathophysiology involves elevated hydrostatic forces, decreased oncotic support, and lymphatic obstruction or insufficiency, with inflammation and tissue remodeling potentially perpetuating fluid retention.

Clinical features

Patients may present with non-pitting or pitting edema of the legs, abdominal distension from ascites, and effusions in the pleural or pericardial spaces. Weight gain, dyspnea, and reduced exercise tolerance can accompany the swelling, with symptoms reflecting the extent and distribution of fluid.
