Delirium, also known as acute confusional state, is a serious disturbance in mental abilities that results in confused thinking and decreased awareness of one's surroundings. It is a transient condition that can develop rapidly, often over hours or days, and can fluctuate in severity. Key features include an altered level of consciousness, such as drowsiness or hyperarousal, and a reduced ability to sustain attention. Disorganized thinking is common, leading to rambling or incoherent speech, and a significant impairment in the ability to think clearly or logically. Perceptual disturbances, such as hallucinations or delusions, can also occur.

Delirium is not a disease itself but rather a symptom of an underlying physical problem. It is frequently associated with infections, especially in older adults, as well as metabolic imbalances, dehydration, medication side effects, drug or alcohol withdrawal, and severe illness or injury. Conditions that affect the brain, such as stroke or head trauma, can also precipitate delirium. Identifying and treating the underlying cause is crucial for recovery. While delirium can be frightening for both the patient and their loved ones, it is often reversible with prompt medical intervention. However, in some cases, severe or prolonged delirium can lead to long-term cognitive deficits.
