Catatonia is a complex psychomotor syndrome that can manifest in various psychiatric and medical conditions. It is characterized by disturbances in motor behavior, volition, and affect. Individuals experiencing catatonia may exhibit a range of symptoms, including immobility, stupor, mutism, posturing, grimacing, stereotyped movements, echolalia (repetition of others' speech), and echopraxia (repetition of others' movements). Conversely, catatonia can also present with agitation, excitement, and excessive motor activity.

The exact cause of catatonia is not fully understood, but it is believed to involve disruptions in neurotransmitter systems, particularly dopamine and serotonin. It is frequently associated with other mental health disorders such as schizophrenia, bipolar disorder, and major depressive disorder. However, catatonia can also be a symptom of medical conditions like encephalitis, metabolic disorders, autoimmune diseases, and certain drug intoxications or withdrawal syndromes.

Diagnosis of catatonia relies on clinical observation and the presence of specific diagnostic criteria, often utilizing scales like the Bush-Francis Catatonia Rating Scale. Treatment typically involves addressing the underlying cause. For severe cases, benzodiazepines are often the first-line treatment, and electroconvulsive therapy (ECT) can be highly effective when other treatments fail or in cases of life-threatening catatonia. Early recognition and intervention are crucial for improving outcomes and preventing complications.
