Antibody-mediated refers to immune processes driven by antibodies, the soluble proteins produced by B cells that recognize specific antigens. This aspect of the adaptive immune system is often described as humoral immunity, in contrast to T cell–mediated (cellular) immunity. Antibodies bind to antigens such as microbes, toxins, or altered host cells, and these interactions can neutralize the threat, mark targets for other immune cells, or trigger inflammatory pathways.

Key effector mechanisms include neutralization, whereby antibodies block the biological activity of pathogens or toxins; opsonization, which coats targets to enhance phagocytosis by macrophages and neutrophils; and agglutination, which clusters particles for easier clearance. Antibodies also activate the classical complement pathway, leading to membrane attack complex formation and cell lysis in some contexts. Fc receptors on natural killer cells, macrophages, and neutrophils mediate antibody-dependent cellular cytotoxicity (ADCC) and antibody-dependent cellular phagocytosis, extending antibody action beyond direct antigen binding.

Antibody-mediated responses are central to protection against many extracellular pathogens and are a primary goal of most vaccines. They also play roles in clinical conditions where antibodies recognize nonpathogenic or self-antigens. In transplantation, donor-specific antibodies can cause antibody-mediated rejection, damaging graft tissue. In autoimmunity, autoantibodies target self-antigens, contributing to diseases such as autoimmune hemolytic anemia, Graves disease, myasthenia gravis, and immune thrombocytopenia. Serological testing for antibodies (titers) helps diagnose infections, monitor immune status, and guide treatment decisions.
