WDEIA
Wheat-dependent exercise-induced anaphylaxis (WDEIA) is a rare form of IgE-mediated anaphylaxis triggered by the combined exposure to wheat ingestion and physical exercise. Symptoms typically occur within about two to four hours after consuming wheat if exercise follows within a few hours, and can include hives, flushing, angioedema, abdominal pain, wheezing, throat tightness, dizziness, and in severe cases shock.
Pathophysiology involves IgE antibodies directed against wheat allergens, with ω-5 gliadin (Tri a 19) repeatedly implicated.
Diagnosis is based on clinical history and supportive laboratory testing for wheat-specific IgE antibodies; component-resolved diagnostics
Management focuses on avoidance: avoid wheat-containing foods before planned exercise and for several hours afterward. Patients
