Vorderhornbereich

Vorderhornbereich, in English the ventral horn region, refers to the anterior portion of the gray matter in the spinal cord. It contains the somatic motor neuron pools that innervate skeletal muscles and is a key component of the motor pathway.

Anatomy and organization: In cross-section the gray matter forms an H-shaped area, with the ventral (anterior) horn occupying the ventral portion. The ventral horn houses motor neurons, largely corresponding to Rexed lamina IX. In the cervical and lumbosacral enlargements, the ventral horn is thick and contains distinct medial and lateral motor columns; the medial column innervates trunk and proximal muscles, while the lateral column supplies distal limb muscles. In the thoracic region the ventral horn is relatively smaller, and a separate lateral horn (intermediolateral cell column) contains autonomic preganglionic neurons, which are not part of the ventral horn proper.

Function: The ventral horn motor neurons project their axons via the ventral roots to skeletal muscles, providing voluntary and reflexive motor control. They receive input from interneurons within the spinal cord and from upper motor centers via pathways such as the corticospinal tract.

Development and variation: The ventral horn develops from the basal plate during embryogenesis. Its size and organization vary along the spinal cord, reflecting the distribution of motor control required for different body regions.

Clinical relevance: Damage to the anterior horn causes lower motor neuron signs, including muscle weakness or paralysis, fasciculations, atrophy, and reduced or absent reflexes. Diseases such as poliomyelitis, spinal muscular atrophy and amyotrophic lateral sclerosis affect ventral horn neurons.
