Therapiesituation

Therapiesituation is a term used in German-language medical documentation to describe the current therapy situation of a patient. It summarizes which therapeutic measures are being applied, planned, or considered, and provides the framework for treatment.

Components include indication, intended start, therapeutic modalities (e.g., pharmacotherapy, psychotherapy, physical therapy, occupational therapy, rehabilitation, supportive or palliative care), duration and frequency, responsible clinicians, treatment goals and expected outcomes. Important aspects include safety considerations such as drug interactions, side effects, and monitoring, as well as patient involvement, consent, and adherence.

Therapiesituation serves planning, coordination, and communication within the care team. It appears in admission and progress notes, handovers, treatment plans, and discharge summaries, and facilitates continuity of care, especially during transitions between settings or specialties.

It is a dynamic state that can change with response to therapy, adverse events, new diagnoses, or changes in patient preferences. Updates are made by clinicians as needed.
