TBspecific

TBspecific is a term used in tuberculosis research and clinical diagnostics to denote tools, assays, and data resources that are specifically indicative of Mycobacterium tuberculosis infection or disease. The label is applied to both laboratory tests and computational models intended to distinguish TB from latent infection and from other respiratory diseases.

In practice, TBspecific can refer to a range of modalities, including gene expression signatures, protein or metabolite biomarker panels, nucleic acid tests optimized for TB, and machine-learning classifiers that integrate clinical and laboratory data to yield a TB-specific score or probability.

Applications include aiding rapid diagnosis in symptomatic individuals, differentiating active TB from other pulmonary pathogens, monitoring treatment response, and supporting epidemiologic surveillance and vaccine research.

Development and validation involve multi-cohort studies to assess sensitivity and specificity, assessments of cross-population performance, and comparison with existing tests such as sputum smear, Xpert MTB/RIF, and interferon-gamma release assays. Regulatory status varies by region and modality.

Limitations include biological and epidemiologic heterogeneity, comorbidities (e.g., HIV), latent infection, prior bacille Calmette-Guérin vaccination, and logistical factors such as cost, required infrastructure, and sample handling. Standardization and transparent reporting are essential for comparability.
