Sundhedsdokumenter

Sundhedsdokumenter are documents that describe an individual’s health status, medical history, treatment, and related information. They may be produced by healthcare professionals such as doctors, nurses, and laboratories, or by patients themselves when reporting symptoms, documenting home measurements, or providing informed consent. Sundhedsdokumenter can exist in electronic form as part of an electronic health record (EHR) or in printed form.

Common types include the patient journal (clinical notes), diagnostic reports (laboratory results, radiology findings), medication lists and prescriptions, treatment plans, discharge summaries, vaccination records, and consent or power-of-attorney forms. Together they form a record intended to support clinical decision-making, care coordination, and legal documentation.

Access to sundhedsdokumenter is governed by data protection and health information regulations. In general, patients have the right to access their own records, and health professionals may share information with other care providers involved in the patient’s care. Data sharing requires lawful purposes and appropriate safeguards, and patients can often control some aspects of their data through consent management.

Digital health infrastructure and interoperability are central to modern sundhedsdokumenter. Electronic systems use standards and coding systems (for example HL7 FHIR for data exchange, ICD-10 or SNOMED CT for diagnoses) to enable consistent sharing. In Denmark, the national eHealth framework and the patient portal Sundhed.dk provide access to parts of health information and facilitate secure communication with healthcare providers.

The quality and privacy of sundhedsdokumenter depend on timely documentation, user awareness, and strict security practices.
