Remodelleringsfaserne refer to the sequential stages that occur during bone remodeling, a continuous physiological process by which bone tissue is renewed and reshaped. The process is essential for maintaining skeletal integrity, repairing microdamage, and regulating calcium homeostasis. It is governed by a tightly coordinated interaction between bone‑resorbing cells (osteoclasts) and bone‑forming cells (osteoblasts).

The first phase is resorption. Osteoclast precursors, which originate from the monocyte‑macrophage lineage, migrate to the bone surface, fuse to form multinucleated osteoclasts, and secrete acids and proteolytic enzymes that dissolve the mineral matrix and degrade the collagen framework. This creates a resorption cavity called a resorption pit. During this phase the bone’s architecture is temporarily compromised, and the release of calcium and phosphate into the bloodstream supplies systemic mineral needs.

Next, a reversal phase occurs, lasting only a few days. Detachment of osteoclasts from the bone surface is followed by the recruitment of pre‑osteoblasts. A specialized cell type called the reversal cell may help prepare the site for new bone formation by removing residual bone debris and producing signaling molecules that activate osteoblast precursors.

The third phase is formation. Osteoblasts lay down new osteoid, a collagenous matrix that subsequently mineralizes. Initially, the newly formed tissue is less dense, but ten to twelve weeks later, the cells become quiescent, and the osteoid thickens and calcifies into mature lamellar bone. Bone matrix proteins such as osteocalcin and osteopontin act as biochemical markers of this process.

Finally, the remodeling phase integrates the previous steps. Simultaneous resorption and formation maintain a balance (homeostasis). Dysregulation of these phases underlies skeletal disorders such as osteoporosis, Paget’s disease, and osteopetrosis. The name 'remodelleringsfaserne' is Danish for 'the remodeling phases', a term commonly used in Scandinavian medical literature to describe this fundamental sequence of bone turnover.
