Mutations can produce distinct outcomes depending on the exact nucleotide change, its position, and the affected protein domain. This mutation-specificity influences enzyme activity, protein interactions, cellular pathways, and responses to therapy. Modern sequencing technologies enable precise identification of mutations, enabling interpretation that goes beyond gene-level categorization.

In clinical practice, mutation-specific information supports diagnostics, prognosis, and treatment selection. Targeted therapies have been developed for particular mutations, and testing often guides choice of drugs or monitoring strategies. Examples include EGFR mutations in non-small cell lung cancer that predict response to EGFR inhibitors, BRAF V600E mutations in melanoma treated with BRAF inhibitors, and KRAS G12C mutations with specific inhibitors. Mutation-specific approaches also extend to hematologic malignancies and inherited disorders, where variant-specific risk assessment or treatment adjustment is used.

Challenges include intratumoral heterogeneity, clonal evolution, and the emergence of resistance mutations that alter sensitivity. Variants of uncertain significance complicate interpretation, and access to high-quality sequencing and expert analysis varies by setting. Ethical considerations and cost-effectiveness also shape implementation.
