Jälkhoitoon

Jälkhoitoon is a Finnish term describing the processes and services that support a person after the initial treatment or discharge from hospital, a care facility, or an acute episode. The aim is to ensure continuity of care, prevent complications, and promote recovery and independence in daily life. Jälkhoitoon encompasses planning that starts during the acute phase and continues in the home or community settings.

The scope of jälkhoitoon is broad and can apply to postoperative care, rehabilitation after illness or injury, chronic disease management, mental health follow-up, and elder or palliative care. Typical elements include discharge planning, follow-up appointments, medication reconciliation, and coordinated access to community services. Rehabilitation therapies (such as physical, occupational, or speech therapy), home care visits, safety assessments, and support for daily activities may be provided in the patient’s home or at outpatient facilities. Education for patients and caregivers, self-management support, and psychosocial assistance are also common components.

Coordination is central to jälkhoitoon. Healthcare professionals—physicians, nurses, therapists, and social workers—work together with primary care providers and municipal or regional services to create and implement a care plan. Patient involvement, informed consent, and privacy considerations are emphasized to tailor the plan to individual needs and preferences.

Outcomes focus on reducing unnecessary readmissions, improving functional status and quality of life, and enabling safe, sustainable living at home. Challenges may include access to services, transportation, and adherence, which require ongoing communication and flexible service delivery.
