Fosforylasbrist refers to a deficiency in the enzyme glycogen phosphorylase. This enzyme plays a crucial role in the breakdown of glycogen, a stored form of glucose, into glucose-1-phosphate. Glycogenolysis, the process of glycogen breakdown, is essential for maintaining blood glucose levels, particularly during periods of fasting or increased energy demand. A deficiency in glycogen phosphorylase can impair this process, leading to a range of clinical manifestations.

There are several types of glycogen phosphorylase deficiency, each affecting different isoforms of the enzyme. The most common forms are linked to deficiencies in liver glycogen phosphorylase (glycogen storage disease type VI, also known as McArdle's disease when affecting muscle) or muscle glycogen phosphorylase (glycogen storage disease type V, also known as McArdle's disease). Symptoms can vary depending on the specific enzyme affected and the severity of the deficiency. Common symptoms include muscle weakness, exercise intolerance, fatigue, and in some cases, liver enlargement and hypoglycemia.

Diagnosis typically involves biochemical testing to measure enzyme activity in muscle or liver tissue, as well as genetic analysis to identify mutations in the relevant genes. Treatment strategies are largely supportive and focus on managing symptoms. This may include dietary modifications to ensure adequate carbohydrate intake and avoid prolonged fasting, as well as physical therapy to manage muscle weakness. For some forms, regular exercise may be beneficial, while for others, it can exacerbate symptoms. The prognosis for individuals with fosforylasbrist depends on the specific type and the effectiveness of management.
