Forvirringslidelse, also known as delirium, is a serious disturbance in mental abilities that can result in confused thinking and decreased awareness of one's surroundings. It typically develops over a short period, often hours to days, and tends to fluctuate. Individuals experiencing delirium may have difficulty paying attention, remembering events, or understanding conversations. Their thought process can become disorganized, leading to rambling or nonsensical speech. Hallucinations, both visual and auditory, can occur, and people may exhibit changes in their sleep patterns, becoming agitated or drowsy at different times.

The causes of delirium are varied and often multifactorial, frequently stemming from underlying medical conditions. Common triggers include infections, especially in older adults, such as urinary tract infections or pneumonia. Dehydration, electrolyte imbalances, and withdrawal from alcohol or certain medications are also significant contributors. Head injuries, strokes, and severe illnesses like sepsis can precipitate delirium. Furthermore, metabolic disturbances, drug interactions, and exposure to toxins are recognized causes.

Diagnosis involves a comprehensive medical evaluation, including a review of the patient's history, a physical examination, and often laboratory tests to identify the underlying cause. Treatment focuses on addressing the precipitating factor. This might involve treating an infection, correcting dehydration, managing pain, or adjusting medications. Supportive care, such as ensuring a safe environment and providing reassurance, is also crucial. While the acute confusion of delirium can be frightening, prompt recognition and management of the underlying cause can often lead to a full recovery. However, in some cases, delirium can be a sign of a more severe underlying illness and may be associated with longer-term cognitive difficulties.
