FiveWhyAnalyse is a problem-solving technique used to explore the cause-and-effect relationships underlying a particular problem. The primary goal is to determine the root cause of a defect or problem by repeatedly asking the question "Why?". Each answer forms the basis of the next question, drilling down into the issue until the fundamental cause is identified.

The technique is attributed to Sakichi Toyoda, who used it at Toyota. It is a simple yet effective tool for root cause analysis, helping to prevent recurrence of problems. When faced with a problem, one starts by stating the issue clearly. Then, the first "Why?" is asked. The answer to this question leads to the next "Why?", and so on. Typically, five iterations of asking "Why?" are sufficient to reach the root cause, though the number can vary depending on the complexity of the problem.

FiveWhyAnalyse is often used in conjunction with other quality improvement and Lean methodologies. Its strength lies in its simplicity and the fact that it does not require complex statistical analysis. However, it is important that the questions are asked by individuals who have a deep understanding of the process or problem being investigated. Misinterpreting answers or stopping the questioning too early can lead to addressing symptoms rather than the true root cause. The technique encourages a deeper understanding of processes and systemic issues.
