Cryopyrin-associated periodic syndrome (CAPS) is a rare group of autoinflammatory disorders characterized by recurrent episodes of fever, skin rashes, and joint pain. The syndrome is caused by mutations in the NLRP3 gene, which encodes a protein called cryopyrin.

The symptoms of CAPS can range from mild to life-threatening and include fevers, chills, muscle and joint pain, and skin rashes. In severe cases, CAPS can lead to amyloidosis, a condition in which abnormal proteins accumulate in organs and tissues, causing damage and dysfunction.

There are four subtypes of CAPS, each with distinct clinical features. The most severe form is neonatal-onset multisystem inflammatory disease (NOMID), which typically begins in early infancy and is characterized by severe joint inflammation and damage to the retina and auditory system.

The diagnosis of CAPS is typically based on a combination of clinical features and genetic testing. Blood tests can help identify the presence of inflammatory markers and may reveal abnormal levels of certain proteins. Imaging studies, such as X-rays and magnetic resonance imaging (MRI), can also be used to evaluate joint damage and other systemic involvement.

Treatment for CAPS usually involves the use of synthetic cytokines, such as anakinra and canakinumab, which can help reduce inflammation and prevent tissue damage. Other treatments, such as corticosteroids and nonsteroidal anti-inflammatory drugs (NSAIDs), may also be used to manage symptoms and prevent complications. With proper treatment, many people with CAPS can experience significant improvement in their symptoms and quality of life.
