Confoundingtekijät, also known as confounding factors, are variables that are associated with both the exposure of interest and the outcome, thereby distorting the apparent relationship between them. Their presence can create a spurious association or mask a real one, complicating causal interpretation in observational studies.

Confounding typically arises when the exposure and the outcome share common causes. For example, age or smoking can confound studies on physical activity or dietary factors and disease risk, because these variables influence both the exposure and the outcome independently of any direct effect.

Confounding is distinct from mediation. A mediator lies on the causal path from exposure to outcome (through which the exposure affects the outcome), while a confounder is not part of that causal path and instead biases the estimate by being related to both exposure and outcome.

Methods to control confounding fall into design and analysis approaches. Design strategies include randomization (in experiments) to distribute confounders evenly, restriction to a homogeneous study population, and matching exposed and unexposed individuals on key confounders. Analysis methods include stratification, multivariable regression adjusting for measured confounders, and propensity score techniques that balance groups. Instrumental variables can address unmeasured confounding under certain conditions, and causal diagrams (DAGs) help identify potential confounders and their relationships.

Limitations include residual confounding from unmeasured or poorly measured variables, time-varying confounding, and overadjustment by controlling for variables that are intermediates. Recognizing and addressing confounding is essential for valid causal inference in epidemiology and related fields.
