Center-specific is an adjective used to describe characteristics, effects, measurements, or procedures that are unique to a particular center or site within a multicenter context. The term is common in research settings where data are collected across multiple locations, and it highlights variation attributable to individual centers rather than the primary variable of interest.

In research, center-specific variation can arise from differences in patient populations, local protocols, staff training, equipment, or follow-up practices. In multicenter clinical trials, failing to account for center-specific effects can confound treatment comparisons and reduce the validity of conclusions. In observational studies, center-specific factors can influence both exposure and outcome, necessitating careful adjustment or stratification.

Modeling approaches include treating center as a fixed effect, which absorbs center-level differences but uses up degrees of freedom, or modeling center as a random effect, which assumes centers are drawn from a common distribution and supports generalization beyond observed centers. Mixed-effects models, hierarchical models, and generalized estimating equations are commonly employed to handle center-specific variation. In meta-analysis, center or site effects may be addressed through random-effects models or subgroup analyses.

In data processing, center-specific calibration or normalization can be used to reduce batch effects when data come from different centers, laboratories, or devices. Examples include multi-site genomics or imaging studies where center-specific biases are adjusted before downstream analysis.

Limitations include potential instability with few centers, overfitting when treating many centers as fixed effects, and modeling assumptions about the nature of center variability. Understanding and addressing center-specific effects improves the accuracy and generalizability of multicenter studies.
