The designation of a treatment line is usually determined by the patient's response to previous therapies, the stage of the disease, and the availability of treatment options.

The first line of treatment is typically the initial therapy prescribed for a newly diagnosed condition. This is often the most standard or potentially curative approach. If the first line of treatment is not effective, or if the disease relapses after an initial period of remission, the patient may move to the second line of treatment. Subsequent lines of treatment, third line, fourth line, and so on, are utilized when earlier treatments have failed.
