Adrenalsparing

Adrenalsparing refers to surgical techniques or medical management strategies aimed at preserving as much functional adrenal gland tissue as possible while treating a disease or condition. This approach is particularly relevant in surgeries involving tumors or other abnormalities of the adrenal glands, which are crucial endocrine organs producing vital hormones like cortisol and aldosterone.

The primary goal of adrenalsparing is to prevent or minimize the need for lifelong adrenal hormone replacement therapy. Complete removal of both adrenal glands, known as bilateral adrenalectomy, necessitates lifelong corticosteroid and mineralocorticoid replacement, which can be complex and carries risks. Therefore, when feasible, surgeons strive to leave behind healthy adrenal tissue, even if a portion of the gland must be removed.

This approach is often considered for conditions such as bilateral adrenal adenomas, certain types of adrenal cortical hyperplasia, and some adrenal metastases. The decision to pursue adrenalsparing depends on factors including the size and nature of the adrenal lesions, the patient's overall health, and the potential for residual tumor recurrence. Minimally invasive surgical techniques, such as laparoscopic adrenalectomy, are frequently employed to facilitate precise dissection and preservation of adrenal tissue. In some cases, medical management may also be used to suppress hormone production from the adrenal glands, thereby avoiding surgery altogether or supplementing any remaining adrenal function.
