Utóterhelés, a term originating from Hungarian, translates to "afterload" in English and is a fundamental concept in cardiac physiology. It refers to the resistance that the left ventricle of the heart must overcome to eject blood during systole, the contraction phase. Imagine the heart as a pump; afterload is the pressure the pump needs to push against to get the fluid out.

This resistance is influenced by several factors. The most significant is the pressure within the aorta, the main artery carrying blood from the left ventricle. Other contributing factors include the elasticity of the arterial walls and the viscosity of the blood. When afterload increases, the heart has to work harder to pump blood. This can be due to conditions like hypertension (high blood pressure), where the aortic pressure is elevated, or conditions that narrow the arteries, such as atherosclerosis.

Chronically elevated afterload can lead to detrimental changes in the heart. The increased workload can cause the ventricular muscle walls to thicken (hypertrophy) as they try to generate more force. Over time, this thickening can lead to impaired relaxation and reduced filling of the ventricle, ultimately compromising the heart's ability to pump efficiently. Conditions like heart failure can be exacerbated or even caused by prolonged high afterload. Conversely, a decrease in afterload, such as with certain medications, can make it easier for the heart to pump blood.
